City Engineer
This municipal recognition program was originally commissioned in 2005 to honor qualifying military service members and veterans with a historical connection to Michigan City. Self-nominations and nominations submitted on behalf of another person are welcome. Posthumous nominations may be submitted by family members or other knowledgeable individuals.
The original 2005 criteria focus on eligible military service on or after September 11, 2001 involving qualifying hazardous-duty or combat-related operations, including Hostile Fire Pay (HFP) or Imminent Danger Pay (IDP), and a qualifying historical home of record or connection to the 938th Police Detachment. Submission does not guarantee an award; the City will verify eligibility and make a final determination under its approved policy.
Please do NOT include a Social Security number, full date of birth, medical information or unredacted military discharge records in this form. If supporting documents are needed, City staff will provide separate secure submission instructions.
A separate recipient/family permission form will be used before any public biography, photograph, or sensitive service details are published. Records submitted to a public agency may be subject to Indiana public records law and any applicable disclosure exemptions.
